At The Reproductive Center, we function as an experienced team dedicated to providing you with an individualized treatment plan. We have a committed staff of skilled and experienced professionals to guide you through the treatment process. We will provide you with the latest advanced technologies and treatment options in a personalized manner in one convenient and personable location. On site is a full service, accredited, state of the art laboratory facility designed for assisted reproductive technologies. We understand the medical, financial, and emotional impact of going through fertility treatments and we are here to help.

If you would like to schedule an appointment, call us at 877-214-8390 (toll-free) or 330-965-8390. You can also email us by clicking on the CONTACT US button and give us your name and phone number and our receptionist will call to schedule your appointment. Once your appointment is made, go to the PATIENT INFORMATION button and click on the new patient and patient history forms. Print these out and complete these forms. Please take the time to give us accurate and thorough information as it is very important in determining your health care needs. Our receptionist will send a new patient packet by mail with additional information requests that we will need at your initial visit. Please return all the forms to us by mail or bring with you at your first visit. We will make a copy of your insurance card and driver’s license at that time. Please arrive 15 minutes earlier than your scheduled time for your first visit.

You can locate our privacy policy under the PATIENT INFORMATION button. You may read and print the policy or you may request a copy of it at the time of your visit. We will ask you to sign a consent and acknowledgment form at your initial visit.
